Residential treatment builds the foundation. Outpatient continues the work in the real world — where the actual challenges of recovery happen. Patients who continue outpatient after residential have significantly better long-term outcomes than those who do not.
A typical post-residential schedule:
- Weeks 1–4 post-discharge — PHP or IOP, plus 12-step/SMART meetings, plus outpatient psychiatric appointments if applicable
- Months 2–6 — IOP tapering to standard OP, ongoing meetings, sponsor work, sober living if needed
- Months 6–12+ — standard OP (1–2x per week), regular meetings, alumni connection, MAT continuation if applicable
- Year 2+ — periodic check-ins, ongoing meetings, independent recovery practice

What a Typical IOP Day Looks Like
Most IOPs run 3 hours per session, 3 evenings per week (typical schedule: 6:00–9:00 PM Monday/Wednesday/Friday). A typical session includes:
- 15-minute check-in and goal review
- 60-minute process group focused on recovery skills, relapse prevention, or specific topics (CBT, DBT, mindfulness)
- 15-minute break
- 60-minute psychoeducation or skills group
- 30-minute closing — homework assignments, weekend planning, accountability
Some IOPs include weekly individual therapy sessions; others coordinate with the patient external therapist. Random drug testing is typically required.
