Heroin withdrawal is severe but rarely directly fatal in healthy adults. The bigger risks are dehydration from vomiting and diarrhea, complications in patients with underlying medical conditions, and post-detox overdose if someone returns to using at their previous dose. A medical detox manages all of these safely.

Both may be considered. Suboxone (buprenorphine) is the most common first-line MAT and can be initiated during detox. Methadone is also effective but typically requires referral to a specialized methadone program. Some patients prefer no MAT and choose comfort-medication-only detox.
