Understanding the BCBS System
Unlike Aetna or Cigna, Blue Cross Blue Shield is not a single company — it is an association of 34 independent insurers that share the Blue Cross Blue Shield brand and participate in the BlueCard network. This means:
- Your specific BCBS plan is operated by your local Blue company (e.g., Blue Cross of California / Anthem, Highmark, CareFirst, Premera, Excellus, Independence Blue Cross)
- Benefits vary significantly between different Blue companies and different plan levels
- The BlueCard program allows you to use your BCBS benefits at providers in other states — which is how many out-of-state patients access BetterChoice in Las Vegas
What BCBS Plans Generally Cover
Most BCBS PPO plans cover the full continuum of substance use disorder treatment:
- Medical detox — covered when medically necessary
- Residential rehab — covered for clinically appropriate stays, typically reauthorized every 7-14 days
- Partial hospitalization (PHP) — covered as step-down or primary level of care
- Intensive outpatient (IOP) — widely covered
- Standard outpatient therapy — covered under behavioral health benefits
- Medication-assisted treatment (MAT) — Suboxone, Vivitrol, and naltrexone generally covered
HMO and narrow-network BCBS plans may have more restrictions.
In-Network vs. Out-of-Network
BetterChoice works with BCBS plans both in-network and out-of-network:
- In-network: contracted rates, lower out-of-pocket costs
- Out-of-network (BlueCard): many BCBS PPO plans cover out-of-network treatment at 60-80% of allowed amounts after the out-of-network deductible
- Single case agreement (SCA): when standard in-network access is not available, we frequently negotiate one-time agreements with the specific Blue company for individual patients
Because each Blue company operates independently, network status can vary by plan. Verification is the fastest way to get a definitive answer.
Prior Authorization
Most BCBS plans require prior authorization for residential substance use treatment. The process varies by Blue company:
- Medical detox (usually 5-7 days approved initially)
- Residential treatment (7-14 days initially, with reauthorization)
- PHP and IOP (varies)
We handle all prior authorization requests. You do not have to navigate the BCBS system yourself.
The BlueCard Program — Out-of-State Patients
If you have BCBS coverage in another state and want to come to BetterChoice in Las Vegas, the BlueCard program typically allows your benefits to be used here. The key details:
- Your home Blue company sets the benefit levels
- The local Blue company (in Nevada) processes the claim
- Out-of-network benefits and deductibles may apply depending on your plan
- We verify BlueCard eligibility as part of our standard benefits check

Anthem BCBS
Anthem is one of the largest Blue companies, operating BCBS plans in 14 states including Nevada. Anthem plans generally provide good coverage for substance use treatment. We have extensive experience working with Anthem specifically. See our Anthem coverage page for details.
Federal Employee Program (FEP)
The BCBS Federal Employee Program covers federal government employees and retirees. FEP Blue Focus and FEP Blue Standard both typically cover substance use disorder treatment, including residential rehab and detox. Verification confirms your specific FEP benefit levels.
How to Verify Your BCBS Plan
Call us at (725) 550-5655 with:
- Your BCBS member ID (the three-letter prefix on your ID card identifies your specific Blue company)
- Group number
- Date of birth
- The name of your specific Blue company (printed on your card)
