What CBT Is
CBT is a structured, present-focused therapy built on a simple insight: how you think shapes how you feel and what you do. When the thoughts running in the background are distorted — catastrophic, all-or-nothing, self-attacking — the feelings and behaviors that follow are often equally distorted. CBT teaches you to notice those thoughts, test whether they are accurate, and choose responses that move your life in the direction you want.
It is not positive thinking. It is accurate thinking, paired with deliberate action.
Why CBT Works for Addiction
Substance use is rarely just about the substance. Behind almost every relapse is a chain of thoughts and feelings that ended at "I need to use." CBT interrupts that chain at multiple points:
- Identifying triggers — the people, places, emotions, and thoughts that precede use
- Challenging cognitive distortions — "one drink won't hurt," "I'll never be okay sober," "everyone else has it together"
- Building coping skills to handle cravings, stress, and high-risk situations without using
- Behavioral activation — restoring activities and routines that produce natural reward
- Relapse prevention planning that you can actually use after discharge
How CBT Is Delivered at BetterChoice

Conditions CBT Treats Effectively
- Substance use disorders (alcohol, opioids, stimulants, polysubstance)
- Depression
- Generalized anxiety, panic, and social anxiety
- PTSD (often with trauma-focused CBT or in combination with EMDR)
- Insomnia
- Chronic pain that complicates recovery
- Anger and irritability
What a CBT Session Looks Like
A typical individual CBT session at BetterChoice is 50 minutes. You and your therapist set a brief agenda for what to work on, review the previous week, work through current thoughts and situations, practice a skill or technique, and set practical between-session work. Sessions are collaborative — you are not lectured at, and you are not asked to dig endlessly into the past. The work is on what is happening now and what you want next.
